To travel to Hungary for up to 90 days, a Schengen visa is required. In addition to all other documents for obtaining a visa, one of the main ones is a visa application form. There is no single standard for the questionnaire, different countries of the Schengen agreement have different forms for applying for a Schengen visa.
Sample of filling out the questionnaire
The questionnaire is filled out in Latin letters. In the paragraphs where it is required to indicate the surname and name, they should be registered identically to the data in the passport. In the paragraph "Surname at birth", if you changed your surname, then indicate the old one, if not, then write as in your passport.
The date of birth is indicated in numbers, the place and country of birth are registered in the same way as in the passport. Then indicate your current citizenship, citizenship at birth, gender, marital status at the time of filling out this application.
Clause 10 is filled out only if this questionnaire is intended for a minor applicant, and then both his parents and their place of registration are registered here. In clause 11, the identification number means the Russian passports of the parents. Further, up to point 16, data from the passport is filled in.
In the paragraph "Home address" you write down the address where you actually live. If you are in another country that is not the country of your citizenship, then in paragraph 18 you must tick the box next to NO. The following paragraphs provide information about your job, position, place of work and the contact details of the employer. Next, fill in the purpose of the trip and indicate the country of destination, or list all countries if you plan to visit several.
Next, indicate the first country where you first crossed the border, indicate the Type of visa, in this case - Schengen. Be sure to indicate the number of days that you plan to stay in Hungary, then fill out the information on Schengen visas for the last three years.
Opposite the point about fingerprints, mark the column NO, that is, their absence. Then the dates of entry and exit into the country are indicated. An entry permit is filled out if necessary. Then you write down the address at which you will live in Hungary, if this is a hotel, then indicate its address and contact information. Next is the point about the host in Hungary - here you need to indicate the person who invited you, or the institution. If there are none, just indicate the hotel in which you will stay.
Then the item is filled in about who pays the costs of your stay in Hungary. And in paragraph 37, and then on the very last page of the questionnaire, the person filling out must put his signature.
Requirements for travel to Hungary
Your passport must be valid with a margin of at least three months from the date of completion of the trip. In addition to the application for a Schengen visa, it is required to fill out a tourist application. You should also prepare in advance 2 color photos, photocopies of Russian passports, documents on financial guarantees.
